Background and Regulation: According to National Health Insurance Act, any foreigner staying in Taiwan with an Alien Resident Certificate (ARC) over six months is obligated to underwrite an insurance policy under NHI. A person staying in Taiwan for full 6 months definition: A person who has, after receiving an ARC, stayed in Taiwan for six consecutive months, or exited Taiwan ONCE for fewer than 30 days within the actual period of stay, amounting to six months after the number of days that he or she has been away from Taiwan would be deducted.
背景與法規：根據《全民健康保險法》，在台灣居住六個月以上且持有外國人居留證（ARC）的外國人有義務根據全民健康保險計劃承保健保。在台灣居住滿6個月的定義：在取得居留證之後連續六個月居留在台灣或六個月期間僅離境台灣1次且少於30天者(離開台灣的天數將被扣除)。

Process of Payment: The cost of National Health Insurance (NHI) should be included in the tuition fee every semester. If you are eligible to underwrite an insurance policy under NHI. The cost is TWD 826 each month (according to the fee criteria set by NHI), so payment per semester is TWD 4,956. Newly arrived students can underwrite an NHI insurance policy only when they have received ARC for six months. Thus, they join the group medical insurance instead at a cost of TWD 500 per month.
費用說明：符合健保資格者，全民健康保險（NHI）的費用將併同學雜費繳交。每月費用為新台幣826元（根據健保局規定辦理），故每學期應付金額為新台幣4,956元。新生取得居留證且連續居留六個月後才能承保全民健康保險，故先投保外籍生傷病醫療險，其費用為每月新台幣500元。

Guide of Insuring in NCYU:

1. For newly arrived freshmen who have NOT been in Taiwan for six months or who have been outside of Taiwan for 30 continuous days in the past six months: Once you have been in Taiwan for more than six months continuously, please follow the procedure outlined in Situation Two to underwrite the NHI policy.
2. For newly arrived freshmen who have been in Taiwan for six months: OIA will enroll you into National Health Insurance and you will see the fee on the billing for next semester. The procedure will take about one week or more, and OIA will inform you to collect the NHI card when we receive it.
3. Students who have underwritten their policy to other associations or schools but want to transfer it to NCYU: Please ask your previous agency (language school, company, or municipal administration, etc.) for an insurance policy closure document. Then please provide it to OIA for insuring.
4. Students who have underwritten their policy to other associations but do NOT want to transfer it to NCYU: Please notify OIA and check the Tuition and Fees bill at the start of every semester to avoid double-paying NHI fees.

嘉義大學投保健保說明：

1. 至台灣未達六個月或在過去六個月內超過30天離境台灣的新生：需符合《全民健康保險法》所述之規定，重新起算連續居留6個月，國際處才能協助申請全民健康保險。
2. 至台灣已經六個月的新生：由國際處協助申請全民健康保險，其費用會併同下學期的學雜費一併繳費。該過程大約需要一個星期或更長時間，當健保局將健保卡寄至國際處時，將另外通知領取。
3. 已有其他單位或他校投保，但須轉由本校投保者：請向之前的代理機構（語言學校、公司或政府相關單位等）索取健保轉出相關資料並繳交國際處，以利協助投保。
4. 已有其他單位投保，無須本校投保者：請在開學前通知國際處並於繳費前檢查繳費單項目，以避免重複支付費用。

NHI Transfer from NCYU:

1. Suspension, withdrawal, or graduation: OIA will withdraw your NHI when students complete the procedure of school leaving.
2. Transferring to another school: Please contact OIA if the school ask you to provide him or her any documents for NHI transferring.

嘉義大學健保轉出說明：

1. 休學、退學或畢業：當完成離校手續後，國際處將辦理健保停保。
2. 轉學：如果新學校要求提供健保轉出相關資料，請與國際處洽詢。

Renewal of NHI IC Card: If you lose or damage your NHI IC card and need to renew it, please apply to the Bureau of National Health Insurance. Renewal costs TWD 200.
健保卡更新：如果您丟失或損壞健保卡，需要更新，請向全民健康保險局申請，其費用為新台幣200。

1. What is work permit？
Should a foreign, overseas Chinese, Hong Kong and Macau student been officially enrolled in a school to take courses in a division, department or graduate institute thereof or to take language course(s) for one year or more, the said foreign student may apply for jobs.
With the exception of the winter and summer vacations, foreign students’ sum of working hours shall not exceed 20 hours per week.

何謂工作證?
來臺正式入學修習科、系、所課程之僑生、港澳生及外國留學生，入學後即可向機關申請，惟來臺學習語言課程之外國留學生，則須修業1年以上，方可提出申請。
工作時間除寒暑假外，每星期最長為20小時。

2. How to apply for work permit？
Foreign students shall submit the work permit application through online application system. (https://ezwp.wda.gov.tw)
Instructions:
Step 1: Please refer to “User Manual “if you are a new user.
Step 2: Select “Work permit for Foreign Students, Overseas Chinese Students and Ethnic Chinese Students” and apply for an account first if you are a new user
Step 3: Documents Required
(1) Original receipt for examination fee: postal receipt, NT $100 per case / Need not attached once entering the data of receipt on the system (or filling in the application form). Nevertheless, it might be requested if necessary)
(2) Photocopy of valid passport permit
(3) Photocopy of student ID card with the enrollment stamp by NCYU Academic Affairs
(4) Photocopy of ARC (front and back)
(5) School Required Documents: NCYU International Student Application for Work Permit with the stamps by adviser, chair of the department and dean of college ,and supporting document.
Step 4: Review and Submit the application

如何申請工作證?

(1) 審查費收據正本:郵政劃撥收據，每一申請案新台幣100元整。於填寫或登錄繳費收據資料後，得予免附，惟必要時本部仍得視個案情形，請學生檢附。
(2) 有效期間之護照影本
(3) 經教務處蓋有在學證明章之學生證影本
(4) 居留證正面影本
(5) 學校要求之文件: 國立嘉義大學外籍生申請工作證證明 (需有指導老師或導師、系主任、學院院長簽章，以及相關證明文件)

保險內容Coverage

門(急)診醫療保險金
被保險人於本契約有效期間內（如於本契約生效後加保之被保險人，則係指加 保之翌日起）因疾病或傷害，而於醫院或診所接受門(急)診診療者，本公司按 醫院或診所實際收取之門(急)診醫療費用（包含診察、處方、醫藥、檢驗或Ｘ 光檢查等之全部費用），給付「門(急)診醫療保險金」，但每次最高給付金額以 1000 元為限。
Outpatient/Emergency treatment benefit
While this Policy is still effective, if the Insured suffers an illness or injury and receives (emergency) treatment at a hospital/clinic as an outpatient, the company will reimburse all medical expenses incurred. However, the payment of benefit shall not exceed the limit of 1,000 NTD (per visit).

每日病房費用保險金
被保險人於本契約有效期間內（如於本契約生效後加保之被保險人，則係指加 保之翌日起）因疾病或傷害而住院診療時，本公司按該被保險人住院期間內所 發生之下列各項費用核付「每日病房費用保險金」，但每日最高給付金額以 1000 元為限：
1.超等住院之病房費差額。
2.管灌飲食以外之膳食費。
3.特別護士以外之護理費。
Daily hospital room benefit
While this Policy is still effective, if the Insured suffers an illness or injury and receives treatment at a hospital as an inpatient, the company will reimburse the following expenses incurred.
1. Balance billing on hospital rooms.
2. Food, except for nutrients provided through tube feeding.
3. Nursing care fee excluding special nurse fee.
However, the payment of benefit shall not exceed the limit of 1,000 NTD (per day).

住院醫療費用保險金
被保險人於本契約有效期間內（如於本契約生效後加保之被保險人，則係指加 保之翌日起）因疾病或傷害而住院診療時，本公司按該被保險人住院期間內所 發生之下列各項費用核付「住院醫療費用保險金」，但被保險人同一次住院最 高給付金額以 12 萬元為限：
1.指定醫師。
2.醫師指示用藥。
3.血液(非緊急傷病必要之輸血)。
4.掛號費及其證明文件。
5.來往醫院的救護車費。
6.手術費用。
7.手術室、手術後恢復室或急救室及其設備之應用。
8.材料費。
9.化驗室檢驗、心電圖、基礎代謝率檢查。
10.復健治療。
11.麻醉劑、氧氣及其應用。
12.放射線診療費。
13.血液透析費。
14.注射技術費及其藥液。
15.檢驗費。
16.治療費。
Hospital miscellaneous benefit
While this Policy is still effective, if the Insured suffers an illness or injury and receives treatment at a hospital as an inpatient, the company will reimburse the following expenses incurred.
1.Designated physician
2.Physician instructions medication.
3.Blood, except for blood transfusions deemed necessary by a doctor providing emergency care.
4.Registration fee and cost of obtaining related certificates.
5.Ambulance fee
6.Operation fee.
7.Operation room, post-operative recovery room, emergency room or and the application of its equipment.
8.Materials fee.
9.Laboratory tests, electrocardiogram, basal metabolic rate check.
10.Rehabilitation therapy.
11.Anesthetics, oxygen, and its application.
12.Therapeutic radiology.
13.Hemodialysis.
14.Injection and its liquid medicine.
15.Laboratory examination.
16.Treatment.
However, the payment of benefit shall not exceed the limit of 120,000 NTD (per hospitalization).

被保險人資格：限外籍生(含陸生及僑生)本人投保。
投保年齡限制：14 歲~50 歲。
本險一律以『記名方式』投保。
保險期間：一年。
繳費方法：年繳、半年繳、季繳及月繳。
Insurer qualifications：Limited to foreign students(including Mainland China students and Overseas Chinese students).
Insured age limit：From 14 years old to 50 years old.
Insured’s name shall be specified in the policy.
Policy period/term：One year.
Premium mode：Yearly, Half yearly, Quarterly , Monthly

如何申請理賠How to make a claim

在保障期內就醫期間，其外籍生傷病醫療保險之理賠，請注意以下事項：
1.保險只承保由保險公司指定醫院提供的醫療。
*如果您正在尋求醫療服務，請務必前往國民健康保險涵蓋的醫院或診所。
2. 支付治療費用後，請務必獲得: 1) 醫療收據正本及醫療明細和 2) 醫療診斷書正本(必須蓋醫院及醫師章)。請告訴醫生您需要這些文件來申請保險理賠。
3.請填寫「國泰人壽保險股份有限公司理賠申請書」，連同所需文件一起提交給國際事務處，本單位人員將協助您送出理賠申請。
*如果您看不懂中文，請將索賠表單帶到國際事務處並請本單位人員協助您填寫。尋求幫助時請遵守應有的禮節。「國泰人壽保險股份有限公司理賠申請書」範本。
4.在接受治療後需要攜帶到國際事務處的必需文件有:
(1) 國泰人壽保險股份有限公司理賠申請書
(2) 醫療收據正本及醫療明細
(3) 醫療診斷書正本(必須蓋醫院及醫師章)
(4) 居留證影本
(5) 在學證明(請先列印學生證正反面並至教務處蓋上在學證明章)
(6) 郵局存簿影本(匯款用)

In order to facilitate your claim after you have purchased the Medical Insurance for international students, please observe the following when seeking medical treatment during coverage period:
1. The insurance covers medical treatment provided by the hospitals specified by the insurance company ONLY.
*Please make sure that you go to one of the National Health Insurance covered hospitals or clinics if you are seeking medical treatment.
2. After paying for the treatment, be sure to get 1) a medical receipt and medical details and 2) medical certificate (must be stamped with hospital and physician seals). Tell the doctor that you need the documents to file for an insurance claim.
3. Fill out the Medical insurance claim form and bring it together with the required documents to the OIA and the staff will assist you to make your claim.
*Bring the claim form to the OIA and ask the staff to help you fill it out if you cannot read Chinese. Observe common courtesy when seeking assistance. Sample of Medical insurance claim form

4. Required documents that you need to bring to the OIA after receiving treatment are:
(1) The insurance claim form
(2) Original medical receipt and medical details
(3) Original medical certificate (must be stamped with hospital and physician seals)
(4) The photocopy of ARC
(5) Enrollment Certificate (The photocopy of Student ID with enrollment seal by Academic Affairs)
(6) The photocopy of Post Office Passbook ( for remittance)
